How Do I Get Support At Home?

The Australian Government updated the aged care program in 2025, which is now called Support at Home. The goal is to make it easier for you to access the care you need, when you need it.
Whether you’re looking for a little help around the house or more hands-on support, the focus is on flexibility, simplicity, and choice.
What is Support at Home?
Support At Home is a new government program designed to replace the current aged care system — including the Commonwealth Home Support Programme (CHSP) and Home Care Packages (HCP) — with a single, streamlined model of care.
The aim? To make it easier for older Australians to live independently at home, with access to tailored services that evolve as your needs change.
Just like before, an aged care assessment will help determine what is available to you as an individual — and your local Care Partner can guide you through the options.
Navigating The Transition
If you’re already receiving care through a Home Care Package or CHSP, your services will continue — and you’ll be supported through the transition to Support at Home.
New to aged care? You’ll now be assessed and connected through the Support at Home framework, ensuring a more personalised and responsive experience.
